<li>I have either been tested (PCR) for COVID-19 and received a negative result within the last 7 days&nbsp;<strong>or</strong>&nbsp;are fully vaccinated as defined by the CDC.</li>
<li>I have not had any signs or symptoms of a fever in the past 24 hours and I do not have any of the following symptoms currently:</li>
